About Enriched Fat Application
What is Enriched Fat Application?
Undoubtedly one of the most interesting developments in Plastic Surgery in recent years is fat injection. Previously, fat injections were thought to serve only as fillers. However, studies have revealed that adipose tissue is the richest source of adult-type stem cells in the body. Thanks to this, the fat that patients once wanted to get rid of has now become a source of healing because of the cells it contains. Stem cells obtained from fat transform into the cell type that is lacking at the site where they are injected and carry out repair activities. For example, when fat is injected into the midface as a filler, the stem cells undergo change, transform into fibroblasts, synthesise collagen and rejuvenate the skin. When stem cell-enriched adipose tissue is applied to the bed of a non-healing wound (e.g. the foot wounds of diabetic patients), it increases vascularisation in the area and contributes to the healing capacity of the tissues.
The adipose tissue taken from individuals is purified by passing it through certain processes, and the stem cells it contains are isolated in concentrated form. This cell cocktail, which also contains these stem cells, can be used as it is to accelerate healing in limited areas, or it can be mixed with more fat to obtain enriched fat. Stem cells are used successfully not only for aesthetic purposes but also in difficult areas such as non-healing open wounds, the foot wounds of diabetic patients and the renewal of skin damaged after radiotherapy.
The new branch of science that studies the use of fat injections and stem cells in both aesthetic and reconstructive surgery has been named Regenerative Medicine. In recent years, the number of publications and the body of knowledge in the medical literature on Regenerative Medicine have been growing rapidly. It appears that in the near future many diseases that are very difficult to treat today may have faster and more permanent solutions with cellular therapies.

Frequently Asked Questions
All QuestionsHow long does fat grafting last?
Since the person's own fat will not trigger any allergic or inflammatory reaction in the body when applied, it lasts longer than other fillers. Although the rates vary according to how the fat is harvested, prepared and applied, part of the applied fat dissolves in the body within a few weeks following the fat grafting procedure. The remaining part, having now become viable in the area where it was applied, stays there for life. As the person gains weight it gains volume, and as they lose weight it shrinks. Since it is the person's own fat, it ages and changes shape with advancing age.
Is it a good thing that the applied fat also ages and changes shape over the years?
In my opinion, the fact that the applied fat ages and can change shape over time should be a desirable feature. Synthetic fillers, which are applied more often than fat, are divided into two types: absorbable and permanent. Although absorbable fillers remain for a certain time after application and provide a volume effect, this effect decreases over time as the body breaks down and removes the filler, and it eventually disappears. For this reason, patients have to have absorbable fillers reapplied at certain intervals. The situation is different with permanent fillers. Once applied, these fillers, which cannot be metabolised and removed by the body, provide volume fullness for a very long time. However, the problem is that even if it looks natural when applied, over time, as the person ages, the filler, which cannot keep pace with this process, continues to give exaggerated volume to the area where it was applied. This causes permanent fillers to lead to an unnatural appearance over time. When the person's own fat is used, however, the aesthetic gain achieved also adapts to the ageing process and to the changes that should occur.
