About Turbinate Surgery

What is Turbinate (Concha) Surgery?

The shape of our nose, which carries significant weight in our face in aesthetic terms, is also a very important organ for our body because of the role it plays in airway physiology and because it forms one of our five senses. Thanks to the septum in the middle and the conchae, also called turbinates, on the side walls of the nasal cavity located at the very beginning of the airway passage, the air we take in through the nose comes into greater contact with the nasal passage and is warmed and humidified, the dust and particles in it are filtered, and part of the inhaled air is directed to the cells through which we perceive the sense of smell, forming one of our five senses.

Anatomically, from bottom to top and from front to back, there are inferior, middle and superior conchae, making a total of six conchae on the two sides. In this order of arrangement, air drawn into the nose first meets the inferior concha.

With the change in volume provided in the conchae

Depending on the condition of the air (its temperature, dryness, and dust and particle content) and on the person's physical activity or psychological state, the inferior conchae swell and their volumes change.

As conchal volume increases, airway resistance increases, and the inhaled air makes more contact with the mucosa and is warmed, humidified and filtered more. The volume of the conchae, which play such an important role in airway dynamics, may increase due to various causes. Turbinate enlargement (conchal hypertrophy) may occur in a person with septal deviation to compensate for the space vacated by the septum shifting to the opposite side, or it may develop secondary to allergy in someone with an allergic constitution.

The treatment of conchal hypertrophy, which, as a result of excessive enlargement, blocks the airway and causes nasal obstruction and mouth breathing, snoring at night and frequent sinusitis attacks, is important for functional nasal surgery. In concha surgery, the aim should not be the complete removal of the concha, which has a functional role, but the opening of the airway.

For this reason, in patients undergoing aesthetic nose surgery and/or correction of septal deviation, if there is accompanying conchal hypertrophy, performing radiofrequency concha ablation to reduce conchal volume, or outfracturing the concha (concha lateralisation) to open the airway, will benefit the patency of the patient's airway.
